SMART provides ADA paratransit in suburban Wayne, Oakland and Macomb counties for people whose disability prevents them from riding its accessible fixed-route buses. The Request for Professional Verification (RPV) is step two of the ADA application. A clinician meets it when a patient applies, and SMART counts the application complete only once both the application and the RPV arrive.
The applicant, family member, friend or professional completes the ADA Application and release. A licensed treating professional, such as a physician, PA, NP, nurse, therapist, social worker, psychologist, optometrist or mobility specialist, completes and signs the RPV with a required license or ID number.

ADA Request for Professional Verification (RPV)
If your copy of the form is a fillable PDF, upload it once and Vero Documents fills it from the visit note and the chart, ready for you to review and sign. Download, print, or email the finished form, with the patient’s consent.
A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.
Vero is not affiliated with, or endorsed by, the agencies and insurers listed here. Read our editorial policy.
No. SMART accepts faxed RPVs only when they come directly from the licensed professional's office with a cover sheet, and faxes from anywhere else are refused. Otherwise the form is mailed to the SMART ADA Office with the application.
It may. Applicants can be asked to complete an in-person functional assessment of travel skills in their neighborhood or a public location such as a community center. The RPV and application come first, and SMART mails its determination letter.
Temporary eligibility is possible when the disability prevents use of fixed-route buses for at least six months. The RPV asks for each condition whether it is temporary and, if so, the date it is expected to end.
