Connecticut B-225T Temporary Disability Parking Placard

Connecticut DMV issues temporary disability parking placards on form B-225T to people whose walking is limited for six months or less. The certifier completes Part B, checking the qualifying criteria from 23 CFR 1235.2 and indicating the duration. Temporary placards carry a fee and, according to DMV, cannot be renewed; a person whose condition is permanent uses form B-225P instead.

Who completes it

The applicant, a guardian or a transporting organization completes Part A and signs under penalty of false statement. A physician, APRN or PA completes Part B for a walking disability; an optometrist, ophthalmologist or the Board of Education and Services for the Blind certifies blindness, and a VA psychiatrist certifies a veteran with PTSD.

Submitting and payment

How to submit it
The applicant returns the original by mail, online, or in person at a DMV branch by appointment only. The form cannot be faxed or emailed, and photocopies are not accepted.
Who pays
The applicant pays DMV a fee for a temporary placard, by check or by credit card in person; DMV sets no fee for the certifier's Part B.

Good to know

  • The certifier must indicate the duration of the temporary condition, which cannot exceed six months.
  • Stamped signatures are not permitted, and the certifier's medical license number and licensing state are required.
  • Qualifying criteria match the renewable placard: unable to walk 200 feet without rest, needing a device, portable oxygen, severe lung or cardiac disease, or arthritic, neurological or orthopedic limits.
  • Only one placard is issued per disabled person. The applicant needs a Connecticut license or ID unless an exception applies, such as an out-of-state resident injured in Connecticut.

Frequently asked questions

How long does a Connecticut temporary disability placard last?

Six months or less. The certifier marks the condition as temporary and indicates the duration in Part B of B-225T. DMV states that temporary placards cannot be renewed, so a longer or permanent condition needs the renewable placard application, form B-225P.

Can I fax the Connecticut B-225T to DMV?

No. The form states it cannot be faxed or emailed and photocopies are not accepted. The applicant sends the original by mail, applies online through DMV's disability placard service, or brings it to a DMV branch by appointment, where temporary transactions are paid by credit card.
