Connecticut B-225P Renewable Disability Parking Placard

Connecticut DMV issues renewable disability parking placards on form B-225P to people who are blind, have a disability that limits walking, or are veterans with PTSD and a walking disability. A qualified certifier completes Part B, checking the criteria from 23 CFR 1235.2 and confirming the condition is permanent, for up to 8 years. Renewable placards are free, and only one is issued per person.

Who completes it

The applicant, a guardian or a transporting organization completes Part A and signs under penalty of false statement. Part B is completed by a physician, APRN or PA for walking disabilities, an optometrist, ophthalmologist or the Board of Education and Services for the Blind for blindness, or a VA psychiatrist for a veteran with PTSD.

Submitting and payment

How to submit it
The applicant returns the original form by mail, online through DMV's disability placard service, or in person at a DMV branch by appointment. DMV no longer accepts placard applications by fax.
Who pays
There is no charge for a renewable placard, and DMV sets no fee for the certifier's Part B.

Good to know

  • Stamped signatures are not permitted, and the certifier's medical license number and licensing state are required. DMV says certification must rest on a genuine professional and patient relationship.
  • Walking criteria include being unable to walk 200 feet without rest, needing a device or another person, severe lung disease, portable oxygen, AHA Class III or IV, or arthritic, neurological or orthopedic limits.
  • The applicant needs a Connecticut license or ID, which must be renewed before the placard. Renewal opens up to 6 months before expiration and runs 60 days after, online or in person.
  • DMV may require additional certification at any time if there is cause to believe the ability to walk is not seriously and permanently impaired. The current version is Rev. 10-2026.
