Washington DOL 420-073: Disabled Parking Application

Washington's Department of Licensing uses form 420-073 for individual disabled parking placards, plates and tabs. A clinician meets it when the healthcare provider section needs a qualifying condition confirmed and the privilege marked permanent (five years) or temporary (up to 12 months). The provider must also write a separate signed authorization on prescription paper or letterhead.

Who completes it

The applicant completes page 1 and chooses placard, plate or tab options. The healthcare provider completes page 2 and signs under penalty of perjury; the signature line accepts only an MD, DO, DC, DPM, ND, ARNP or PA.

Submitting and payment

How to submit it
The applicant takes or mails the form and the provider's separate authorization to a county auditor or vehicle licensing office, or to DOL's Applications & Issuance unit; temporary applications can also be filed online.
Who pays
Temporary and permanent placards carry no fee; permanent plates and disabled parking tabs require DOL fees.

Good to know

  • The separate authorization must state the applicant's name and that they have a qualifying condition. Printing 420-073 itself on prescription paper satisfies both requirements.
  • Qualifying conditions include acute sensitivity to auto emissions, legal blindness with limited mobility, and porphyria, alongside the usual walking, lung and cardiac criteria.
  • Knowingly false information is a gross misdemeanor, and the practitioner may also face sanctions under chapter 18.130 RCW, the Uniform Disciplinary Act.
  • Related forms: 420-076 for replacements, 420-529 for organizations and 420-105 for veterans.

Frequently asked questions

What is the separate authorization for Washington disabled parking?

A signed note from the provider, on prescription paper or office letterhead, stating the applicant's name and that they have a condition qualifying them for disabled parking privileges. DOL requires it in addition to the form. If the provider prints 420-073 on prescription paper, the form meets both requirements.

How long does Washington disabled parking last?

The provider decides. Temporary privileges last up to 12 months, with one placard issued and a new application needed to renew. Permanent privileges are valid for five years, and DOL sends a renewal notice before they expire. Permanent plates or tabs require the applicant to own the vehicle.

Who can sign Washington form 420-073?

The signature line is limited to an MD, DO, DC (chiropractor), DPM (podiatrist), ND (naturopath), ARNP or PA. The signer declares under penalty of perjury that the applicant has a medical necessity that severely affects mobility or involves acute sensitivity to light, as RCW 46.19.010 requires.
