Iowa Form 411055 Disability Parking Medical Statement

Iowa residents use form 411055 to apply for a persons with disabilities removable windshield placard, sticker or plates. The form itself has no clinician section: the applicant attaches a signed medical statement on the practitioner's letterhead. A clinician meets this request when a patient's walking is limited enough to qualify and the Iowa DOT needs to know whether the disability is permanent or temporary.

Who completes it

The applicant completes and signs the application. A physician (including a podiatrist), chiropractor, physician assistant, nurse practitioner, occupational therapist or physical therapist licensed in any U.S. state writes and signs the separate statement.

Submitting and payment

How to submit it
The applicant uploads the statement with the online application, mails both to Iowa DOT Vehicle Services, or brings them to an Iowa DMV location or county treasurer, where placards can be issued immediately.
Who pays
Iowa charges nothing for the placard, sticker or standard plates (regular registration still applies); Iowa DOT does not address payment for the practitioner's statement.

Good to know

  • Qualifying limits include being unable to walk 200 feet without resting, needing a brace, crutch or other assistive device, using portable oxygen, or AHA Class III or IV cardiac disease.
  • A temporary placard expires after six months unless the practitioner gives an earlier date, so the statement should name an end date when recovery is expected sooner.
  • Permanent placards last five years. Plates and stickers go only to owners with a permanent disability, and plates are revalidated each year by the owner's self-certification.
  • A Department of Veterans Affairs determination of a service-related disability can be submitted instead of a practitioner's statement.

Frequently asked questions

What must the Iowa disability parking medical statement say?

Iowa DOT asks for a statement on the practitioner's letterhead, signed by the practitioner, that includes a determination of whether the disability is permanent or temporary. For a temporary condition, the practitioner may set an expiration date earlier than the default six months.

Can an out-of-state provider sign an Iowa disability parking statement?

Yes. Iowa accepts statements from physicians (including podiatrists), chiropractors, physician assistants, nurse practitioners, occupational therapists and physical therapists licensed in any U.S. state, which helps residents treated across state lines. The applicant must still be an Iowa resident.

How long is an Iowa disability parking placard valid?

A temporary placard expires six months from issue, or earlier if the medical professional indicates a shorter period. A permanent placard expires five years from issue. Stickers last as long as the vehicle registration is current.
