Frontier Form 30881: POC Medical Authorization

Frontier Airlines requires passengers who want to use a portable oxygen concentrator on board to carry this physician's statement, or a written statement answering the same questions. The physician confirms the patient can see, hear and respond to device alarms, whether oxygen is needed throughout the trip, and the maximum flow setting at an 8,000-foot cabin altitude. Agents and crew review it at the airport.

Who completes it

The passenger's physician completes the Physician's Statement: four questions, patient name, date, physician name, telephone and signature. The passenger keeps the original; customer service agents review it, and the flight attendant tells the pilot in command a POC is on board.

Submitting and payment

How to submit it
Nothing is sent to Frontier ahead of time: the passenger brings the signed form or physician statement to the airport and shows it to customer service representatives and flight crew before travel.
Who pays
Frontier carries a POC free as an assistive device, and the form sets no fee for the physician.

Good to know

  • The form is valid for one year from the date of the physician's signature.
  • Only FAA-approved POC models may be used on board; units without the red-lettered label or not on Frontier's approved list can be carried but not used.
  • If oxygen is not needed at all times, the physician lists the portion of the trip not requiring oxygen and the maximum time the patient can go without it.
  • Passengers must carry enough charged batteries in carry-on bags for the whole trip, including connections and delays; Frontier advises planning for at least 150% of flight time.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

How long is Frontier's POC medical authorization valid?

One year from the date the physician signs it. A passenger who flies Frontier more than once in that year can reuse the same signed form, as long as the answers still reflect the patient's oxygen needs and device. After a year, the physician completes a new one.

Can a doctor write a letter instead of Form 30881?

Yes. The form tells the physician to either complete it or give the passenger a written statement with the same information: ability to see, hear and understand alarms, ability to act on them without help, whether oxygen is needed at all times, and the maximum flow setting at 8,000 feet cabin altitude.

What flow setting does the physician give on Form 30881?

Frontier's pressurized cabin altitude equals 8,000 feet above sea level. The physician enters the maximum flow setting the patient may adjust to as needed in flight, recognizing that cabin pressure can change. The form also asks for the type of unit carried.
