South Dakota Form 1305: Disabled Parking Permit, Plates

South Dakota's Form 1305 is the single application for disabled person portable parking permits and disabled person license plates. A licensed physician meets it on page 2, the medical certification, which confirms a physical disability that limits or impairs walking. The applicant completes it as a web form, prints it, and files it with the county treasurer.

Who completes it

The applicant completes the personal and vehicle sections. A licensed physician completes and signs the page 2 medical certification, identifying the qualifying criterion and whether the disability is permanent or temporary (up to 12 months).

Submitting and payment

How to submit it
The printed form goes, with any fees, to the county treasurer of the applicant's county of residence; the Department of Revenue does not process electronic submissions.
Who pays
South Dakota charges nothing for disabled parking permits and adds no charge for disabled person plates.

Good to know

  • Criteria: cannot walk 200 feet without resting, needs an assistive device or another person, lung disease below set thresholds, portable oxygen, AHA Class III or IV cardiac condition, or arthritic, neurological or orthopedic limits.
  • Blindness or impaired vision by itself is not a valid criterion for a South Dakota disabled parking permit or plate.
  • Permanent permits renew every five years without a new doctor certification; temporary permits need a new certification for each renewal.
  • A permit must be surrendered to the county treasurer within 30 days once the holder is no longer disabled or has died. Disabled veteran plates use form MV-4000.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Where do South Dakota patients file Form 1305?

With the county treasurer in the applicant's county of residence. The Department of Revenue posts Form 1305 as an online form, but it must be printed, signed by the physician and submitted on paper with any fees; electronic submissions are not currently processed.

Does a South Dakota permanent parking permit need a new doctor's signature to renew?

No. Permanent permits renew every five years without a new physician certification. Temporary permits, which last up to 12 months, are different: each renewal needs a new certification from the physician, so patients with a longer temporary condition return for another page 2.

Is there a printable PDF of South Dakota Form 1305?

The Department of Revenue publishes Form 1305 as a web form rather than a static PDF; the applicant fills it in online and prints it. Parts of the department's page also refer to the plate application as MV-044, so patients may arrive with either number.
