Colorado M-11: Comprehensive Evaluation

When a person is on a Colorado emergency mental health hold (M-1), the evaluating facility documents its assessment on this form to decide whether the person needs further treatment, will accept it voluntarily, or should be certified. It records the presenting problem, suicide and violence risk, grave disability, access to lethal means, treatment history, psychosocial factors, collateral information and the statutory certification factors.

Who completes it

An evaluating professional completes and signs it, with a co-signature line. Non-designated emergency medical services facilities complete only Section I; designated facilities complete the entire form.

Submitting and payment

How to submit it
The facility keeps the evaluation in the person's record and may build its requirements into the EHR instead of using the form. A certification recommendation leads to the Notice of Certification (M-12).

Good to know

  • Section I covers clinical presentation and safety. Section II adds treatment history, substance use, medications, current providers, psychosocial history, legal concerns and collateral information.
  • The certification factors page covers insight, diagnosis and prognosis under C.R.S. 27-65-108.3(2), recent overt acts, prior patterns of deterioration and ability to meet basic needs.
  • If the evaluator is not a professional person and recommends certification, they notify the facility, because only a professional person may initiate certification.
  • If no further treatment is needed, the disposition calls for a safety plan. Current edition Rev. 07/2026, eight pages, with a box to attach any psychiatric advance directive.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Do emergency departments have to complete the whole M-11?

No. The form states that non-designated emergency medical services facilities only have to complete Section I, including its disposition and transfer details to a designated facility. Other facilities initiating or receiving a person on an M-1 hold complete the entire form.

Can the M-11 be completed in the EHR?

Yes. The form says both emergency medical services facilities and designated facilities may incorporate its requirements into their electronic health record instead of completing the form directly, as long as the same information is captured for each person on a hold.

What does the M-11 disposition decide?

It records the diagnostic impression (danger to self, danger to others, gravely disabled), whether further treatment is needed, and whether the person will accept it voluntarily. If not, certification is completed; if no treatment is needed, a safety plan is completed.
