Colorado M-1: Emergency Mental Health Hold

In Colorado, an intervening professional or peace officer completes this application to take a person into custody for up to 72 hours of treatment and evaluation when the person appears to have a mental health disorder and, as a result, to be an imminent danger to self or others or gravely disabled. It documents presenting symptoms, orientation, access to weapons and a narrative supporting probable cause.

Who completes it

The intervening professional or peace officer completes and signs the whole form, with title, agency and Colorado license or badge number; a co-signer line is provided. The individual does not complete any part.

Submitting and payment

How to submit it
The application accompanies the person to the facility named for the 72-hour evaluation. Required copies go to the receiving facility, the individual and records.

Good to know

  • Symptom checklists cover physical activity, emotional reaction, dress and hygiene, speech, and expressions such as suicidal or homicidal thoughts or hearing voices.
  • The form asks whether the person is responsible for other people, pets or property, such as temporary housing, that the hold may put at risk.
  • The notice to the individual states that evaluation and treatment may not exceed 72 hours, and a line records when the hold is discontinued.
  • Current edition Rev. 07/2026. BHA posts fillable M-forms, including a single-page M-1, in a Google Drive folder linked from its M-forms page.
