JV-220(B): Request to Continue Medication (CA)

When a California dependent or ward's psychotropic medication authorization needs renewing with no change in drug or dose, the prescribing physician can use this shorter attachment instead of JV-220(A). It asks the juvenile court to continue the same medications at the same maximum doses, with an updated assessment of the child's mental health, response to medication, diagnoses, medical history and labs.

Who completes it

Only the prescribing physician who completed the most recent JV-220(A) can complete and sign it. The social worker or probation officer completes the JV-220 application.

Submitting and payment

How to submit it
Give it to the social worker or probation officer, who files it with the Application for Psychotropic Medication (JV-220).

Good to know

  • Both boxes in item 2 must apply: same medication and maximum dosage, and same prescribing physician as the last JV-220(A). Otherwise, complete JV-220(A).
  • The request still rests on a dated face-to-face clinical evaluation, and the side-effect, interaction and withdrawal information for each medication must be attached.
  • Each approval runs for the listed duration or six months from the order, whichever comes first. Alternative mandatory form under rule 5.640(c)(7), revised January 1, 2018.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

When can JV-220(B) be used instead of JV-220(A)?

Only when the request is to continue the same psychotropic medication at the same maximum dosage the child is currently taking, and the same physician who completed the most recent JV-220(A) is making it. If either is not true, use JV-220(A).

What does JV-220(B) still require?

A face-to-face evaluation date, the child's current height and weight, an assessment of overall mental health and response to medication, DSM-5 diagnoses, relevant medical history and labs, whether the child and caregiver were informed, planned therapeutic services, and each medication's schedule and maximum daily dose.

Who files JV-220(B) with the court?

The prescribing physician completes and signs it, then gives it to the child's social worker or probation officer, who files it with the Application for Psychotropic Medication (JV-220). Form JV-217-INFO explains the full set of psychotropic medication forms.
