New Jersey CN 12042: Certification of Examining Prof.

New Jersey guardians of the person may have to file a periodic Report of Well-Being, and the CN 12042 is the clinician's part of it. A clinician meets it when a guardian brings the form after a recent medical evaluation of the incapacitated person. The certification states whether the person's condition has changed during the reporting period and whether the guardianship should continue unchanged or be modified.

Who completes it

The guardian fills in their own name, address and phone number and the incapacitated person's full legal name at the top. A medical professional such as an MD or DO who recently evaluated the person completes the rest and signs.

Submitting and payment

How to submit it
The clinician returns the signed certification to the guardian, who files it with the Report of Well-Being in the Superior Court.

Good to know

  • A clinician may use their own letter or form instead, as long as it addresses all of the information the CN 12042 asks for.
  • The professional chooses between two findings: condition essentially unchanged, or changed during the reporting period, with a description of the change.
  • If capacity has changed, the professional describes how the guardianship should be modified; extra pages may be attached when more space is needed.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Can a doctor use their own letter instead of the CN 12042?

Yes. The instructions say that if the examining professional wishes to use their own form, the guardian should make sure the statement addresses all of the information in the CN 12042: the examination date and place, any change in condition, and an opinion on continuing or modifying the guardianship.

Who fills out the CN 12042?

The guardian completes only the header with their contact details and the incapacitated person's name, then gives the form to a medical professional, such as an MD or DO, who has performed a recent evaluation. The professional completes and signs the certification.
