Colorado Hearing Impairment Rating Worksheet (DOWC)

Colorado Level II accredited physicians use this one-page worksheet to compute hearing loss for a workers' compensation permanent impairment rating. Thresholds at 500, 1000, 2000 and 3000 Hz are summed for each ear, converted to binaural impairment with tinnitus added, then to whole person impairment, with a separate monaural result for each ear.

Who completes it

The Level II accredited physician performing the rating, whether the authorized treating physician or a Division IME physician, completes it from audiometry. There is no section for the injured worker.

Submitting and payment

How to submit it
Attach it to the impairment rating report. The Division's report checklist asks raters to complete their worksheets and include them with the report.
Who pays
The worksheet carries no separate fee or billing code.

Good to know

  • In Section 2 the worse ear's total goes on the vertical axis and the better ear's on the horizontal axis of Table 3; totals under 100 dB are entered as 100.
  • Tinnitus is added to the binaural hearing impairment before the total is converted to whole person impairment with Table 4.
  • Colorado Rule 12 requires all permanent impairment ratings to be based on the AMA Guides, Third Edition (Revised), and final percentages reported in whole numbers.
  • The Division posts this worksheet with its other rating worksheets on the Impairment Rating and Treatment Resources page.
