Colorado WC132: DIME Examiner's Summary Sheet

Colorado Division Independent Medical Examination (DIME) physicians record their conclusions on this summary sheet: whether the claimant is at maximum medical improvement and the date, and impairment ratings for the spine, each extremity converted to whole person, psychological and other impairment, the final combined unapportioned rating and, if applicable, an apportioned rating.

Who completes it

The DIME physician completes and signs it. The DIME Unit pre-fills the sheet with the case details and sends it to the physician.

Submitting and payment

How to submit it
Within 20 days of the exam send it to all parties listed on the sheet and to the DIME Unit, in this order: invoice, summary sheet, WC201 report, then all applicable worksheets.
Who pays
The DIME fee is billed to the insurer on total time in 15-minute increments, minus the base fee already paid; time revising an incomplete report is not billable.

Good to know

  • If apportionment is performed, Desk Aid 14, the apportionment calculation worksheet, must be attached; page 2 shows the apportionment algorithm for injuries before and after July 1, 2008.
  • Address every issue on the DIME application; the DIME Unit sends an Incomplete notice if the report has errors and a Concluded notice when it is complete.
  • If the claimant is not at MMI, the physician gives a provisional rating and report, and a follow-up DIME takes place after the recommended treatment.
  • Revision 01/20; the Division also lists it as the Independent Medical Examiner's Summary Sheet.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

When is the Colorado DIME report due?

Within 20 days of the examination. The DIME physician sends the invoice, WC132 summary sheet, WC201 report and all applicable impairment rating worksheets, in that order, to every party listed on the summary sheet and to the DIME Unit.

What if the claimant is not at MMI at the DIME?

Check 'No' on item 3 and provide a provisional impairment rating and report, with the treatment recommended to reach MMI. A follow-up DIME is scheduled after the worker completes that treatment and is placed at MMI again by a treating provider.

What must be attached if the DIME physician apportions?

Desk Aid 14, the Apportionment Calculation Worksheet, is required whenever apportionment is performed. The apportioned final combined rating goes in item 5, separate from the unapportioned rating in item 4.
