Colorado WC164 Physician's Report of Injury

Colorado's Rule 16 requires this two-page report from the authorized treating physician on every workers' compensation injury. It is marked initial at the first visit, progress when the condition, diagnosis or treatment changes, and closing at MMI, when a rating is needed or at discharge. It covers diagnosis, treatment plan, work status and restrictions, referrals, MMI, maintenance care and permanent impairment.

Who completes it

The authorized treating physician completes all applicable fields and signs with license number. The injured worker's own description of the accident is recorded at the initial visit.

Submitting and payment

How to submit it
Send a copy to the injured worker and to the insurer, with the narrative report attached; the narrative does not replace the form.
Who pays
The form itself does not list a fee or billing code.

Good to know

  • Restrictions are marked temporary or permanent, with lifting, carrying and pushing limits in pounds and hours per day for walking, standing, sitting, kneeling and similar activities.
  • Discharging a worker for non-compliance or nonmedical reasons requires written notice by certified mail to the worker and insurer within 3 business days (see Desk Aid 15).
  • Item 10, permanent medical impairment, is required; a rating must come from a Level II accredited physician, with the Division's worksheets and a narrative attached.
  • MMI is defined on the form as the point when impairment is stable and no further treatment is reasonably expected to improve it. Revision 02/19.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Vero is not affiliated with, or endorsed by, the agencies and insurers listed here. Read our editorial policy.

Frequently asked questions

When is WC164 marked Initial, Progress or Closing?

Initial for the first visit for the described injury. Progress when a change in condition, diagnosis or treatment occurs. Closing when the injured worker is at MMI, needs an impairment rating, or is discharged from care. The exam date is the date of the office visit.

Does my narrative report replace Colorado WC164?

No. The instructions say to complete all applicable fields and attach your narrative report to support the findings; the narrative does not replace the form. When an impairment rating is given, attach the Division's required worksheets and a report describing the rating.

Who can give the impairment rating on WC164?

If the injury will cause permanent impairment, a Level II accredited physician must perform the rating. A treating physician without that accreditation marks the box for referral to a Level II physician and lists the referral in item 7b.
