Colorado WC201: DIME Report Template

Colorado Division Independent Medical Examination physicians must write their report on this template, and every topic is required. It runs from the scope of the exam and injury history through records review, examination, testing, psychological evaluation and diagnosis to the MMI date, the impairment rating under the AMA Guides Third Edition (Revised), the rationale, work restrictions, maintenance care and a signed attestation.

Who completes it

The DIME physician, who is Level II accredited, completes all sections A through O and signs the attestation.

Submitting and payment

How to submit it
Within 20 days of the exam, send it to all parties listed on the summary sheet and to the DIME Unit, after the invoice and WC132 summary sheet and followed by all applicable worksheets.
Who pays
Section O attests that billed charges comply with C.R.S. 8-42-101(3)(a)(I), Rule 16 and Rule 18 and records the total minutes spent on the DIME.

Good to know

  • Section K: match each work-related diagnosis to the AMA Guides, state pages and tables, attach worksheets, rate at extremity and whole person level, and address every body part on the DIME application.
  • Section J sets a final MMI date covering all areas under treatment and addresses differing MMI dates; if not at MMI, describe the treatment needed under the guidelines.
  • Section L explains the reasoning and addresses any rating differences between providers.
  • Records the worker brings to the exam should not be reviewed; note in the report that they were not examined. Revision 6/26.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Is the WC201 template mandatory for Colorado DIME reports?

Yes. The template states that it is to be used when writing DIME reports and that all topics are required. Reports with errors or missing topics get an Incomplete notice from the DIME Unit and must be revised and resubmitted to all parties and the Unit at the same time.

What goes in the impairment rating section of WC201?

Section K matches each work-related diagnosis to the AMA Guides, Third Edition (Revised), states the pages and tables used, includes all worksheets, rates at extremity and whole person level, and covers every body part on the DIME application. If apportioning, give the prior rating and attach the calculation worksheet.

Is time spent revising a DIME report billable?

No. The DIME Physician Quick Reference Guide says time spent revising a report after an Incomplete notice is not billable. Billable time runs from physician confirmation until the report is submitted, in 15-minute increments, and Section O records total minutes.
