DWC Form PR-4: Primary Treating Physician P&S Report

California primary treating physicians use PR-4 to report the initial evaluation of permanent disability once a patient is permanent and stationary, for ratings under the 2005 Permanent Disability Rating Schedule. It reports whole person impairment under the AMA Guides 5th edition with tables and pages, any pain add-on, apportionment, future medical treatment and a functional capacity assessment.

Who completes it

Only the primary treating physician completes and signs it; a QME or AME must not use it to report a medical-legal evaluation.

Submitting and payment

How to submit it
Send it to the claims administrator once the condition is permanent and stationary; for injuries on or after 1/1/13 attach DWC-AD 10133.36.
Who pays
PR-4 is billed under the California-specific Official Medical Fee Schedule code WC004 (8 CCR 9789.14).

Good to know

  • For each impairment give the WPI percentage, the table and page numbers used and an explanation of how it was derived; the form fits up to 12 ICD-10 diagnoses.
  • Pain impairment beyond what the AMA Guides chapters already include can add 0 to 3 percent WPI, and the total for a single injury cannot exceed 3 percent.
  • Apportionment is based on causation under Labor Code 4663 and 4664; a report without an apportionment determination is not complete on permanent disability.
  • Sign with an original signature, not a stamp, declaring under penalty of perjury that Labor Code 139.3 was not violated. The current revision is 02/2016.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

What if I cannot determine apportionment on the PR-4?

State the specific reasons you could not make the determination. Under Labor Code 4663(c), as reprinted on the form, the physician then consults other physicians or refers the employee to another authorized physician so the final apportionment determination can be made.

Does the functional capacity section of PR-4 affect the rating?

No. The form says the functional capacity assessment is prepared only to decide whether the claimant can return to the usual occupation and is not considered in the impairment rating. For injuries on or after January 1, 2013, DWC-AD 10133.36 must also be completed.

Who can complete the PR-4?

Only the primary treating physician. QMEs and AMEs report medical-legal evaluations differently. The PR-4 is completed when the patient has residual effects or may need future care, once the condition is permanent and stationary, and is signed with California license number and specialty.
