DWC Form 9783: Predesignation of Personal Physician

California employees who want their own doctor to treat a future work injury give this notice to the employer before any injury happens. It names the personal physician or medical group and the employee's nonoccupational health plan. The physician's signature confirms agreement to be predesignated; without it, other proof of that agreement is needed under 8 CCR 9780.1(a)(3).

Who completes it

The employee completes and signs the notice section. The physician, or a designated employee of the physician or medical group, signs the line reading 'Physician: I agree to this Predesignation'.

Submitting and payment

How to submit it
The employee gives the signed notice to the employer before any work injury occurs.
Who pays
The form carries no fee or billing code; the physician's only task is the agreement signature.

Good to know

  • On the injury date the employee must have health coverage for nonoccupational injuries or illnesses, and the doctor must be their regular physician who has directed their care and keeps their records.
  • A regular physician must limit practice to general practice or be a board-certified or board-eligible internist, pediatrician, OB-GYN or family practitioner.
  • A medical group qualifies only as a single corporation or partnership of MDs or DOs running an integrated multispecialty group that mainly treats nonoccupational conditions.
  • A personal chiropractor or acupuncturist is predesignated on a different form, DWC 9783.1, which has no provider signature line. The current form is dated 7/2014.

The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Does the doctor have to sign DWC Form 9783?

No, the physician is not required to sign. But if neither the physician nor a designated employee of the physician or medical group signs, other documentation of the physician's agreement to be predesignated is required under Title 8, California Code of Regulations, section 9780.1(a)(3).

Is DWC Form 9783 available in Spanish?

Yes. DWC publishes a Spanish version titled Designación previa de médico personal alongside the English form. Both carry the same eligibility conditions, employee notice section and physician agreement line, so an employee can complete whichever version they read best.

Who can be predesignated on Form 9783?

A personal medical doctor (MD), doctor of osteopathic medicine (DO) or qualifying medical group. The doctor must be the employee's regular physician, must have directed their treatment and keep their records, and must agree before the injury to treat work injuries or illnesses.
