DWC-AD 101: Request for Summary Rating of QME Report

When a California injured employee has no attorney and no application for adjudication, the claims administrator uses this form to ask the Disability Evaluation Unit to rate the panel QME's report. It goes to the QME with all medical reports and records before the exam, and the QME sends it back to the DEU as the cover sheet for the finished evaluation.

Who completes it

The claims administrator completes the claim, employer, employee, occupation and earnings details and serves the form on the employee. The QME adds the physician name and exam date.

Submitting and payment

How to submit it
The QME submits the evaluation and the employee's DEU Form 100 to the listed DEU district office with this form as cover sheet, and serves copies of the report and Form 100 on the claims administrator and employee.
Who pays
The form does not address the evaluator's fee.

Good to know

  • In an unrepresented case the QME must review and comment on the Employee's Disability Questionnaire (DEU Form 100) in the report, and give the employee a blank one if they arrive without it.
  • The claims administrator must serve the form on the employee before the evaluation and complete the proof of service on page 4.
  • Page 1 records the date of the first medical report showing permanent impairment and the last date temporary disability was paid.
  • Weekly earnings are rated at the maximum unless a detailed earnings statement is attached. The current form is REV. 11/2008.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Who fills out DWC-AD 101?

Mostly the claims administrator, who completes the claim, employer, employee, occupation and earnings sections, sends it with all medical records to the evaluating physician, and serves it on the employee. The QME's part is small: the physician name and exam date, then using the form as the DEU cover sheet.

What does the QME do with DWC-AD 101?

The QME submits the completed medical evaluation, together with the employee's DEU Form 100 when the employee is unrepresented, to the Disability Evaluation Unit district office named on the form, using DWC-AD 101 as the cover sheet. Copies of the report and Form 100 go to the claims administrator and the employee.

When is DWC-AD 101 used?

Only when the injured employee is unrepresented and has not filed an application for adjudication; the form's instructions limit it to those cases. It must reach the evaluating physician, with the medical records, before the scheduled examination.
