WSCC PT/OT Functional Abilities Report Form C (CS113)

Physiotherapists and occupational therapists in the Northwest Territories and Nunavut use this two-page report to measure a WSCC claimant's physical abilities against the demands of their pre-injury job. It tracks lifting, carrying, pushing, pulling, standing, sitting, walking and grip strength across up to four test periods, then records work capability, overall progress and the final return-to-work outcome at discharge. Copies go to the physician, the employer and the WSCC.

Who completes it

The physiotherapist or occupational therapist completes the testing and the whole form, initialling each test period, and records whether weights and times were reported by the worker, the employer or another source. The worker and employer do not complete any section.

Submitting and payment

How to submit it
Email or fax it to WSCC's NWT or Nunavut claims department as printed on the form, with copies to the worker's physician and employer, as the form states.
Who pays
Bill testing separately on the WSCC Medical Invoice form, sent to Financial Services within one year of the service, as WSCC returns invoices in other boards' formats.

Good to know

  • Lifting and carrying are recorded in pounds or kilograms as frequent (66 percent) or occasional (33 percent), and tolerances as reported or observed.
  • Each period records work capability as pre-injury or modified duties and progress as improving, no change or declining; modified duties need a return-to-work or stay-at-work plan.
  • The final return-to-work outcome is completed at discharge: no time lost, pre-injury duties, suitable duties with a date, or did not return with a reason.
  • Form A (CS111) and Form B (CS112) both ask whether Form C is attached, and want a reason when it is not. The printed revision is CS113 0824.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

Frequently asked questions

What does WSCC Form C measure?

Lifting above shoulder, horizontally and floor to waist; carrying with right, left or both hands; pushing and pulling; standing and sitting in minutes; walking distance; and right and left grip strength. A column for pre-injury job demands lets the WSCC compare each result with what the worker's job requires, and other essential job tasks can be added.

Who receives a copy of WSCC Form C?

The form is marked as copied to the physician, the employer and the WSCC. Its work capability section shows whether the worker can do pre-injury or modified duties in each test period, and when modified duties are selected, the form asks for a return-to-work or stay-at-work plan.

Is WSCC Form C available in French?

Yes. WSCC publishes CS113 as a fillable PDF in English and in French, and the English form notes that a French version is available. The same form serves workers in both the Northwest Territories and Nunavut, and it sits with Form A (CS111) and Form B (CS112) on WSCC's provider forms page.
