WSCC PT/OT Progress Report Form B (CS112)

After the initial assessment on Form A, physiotherapists and occupational therapists treating WSCC claimants in the Northwest Territories and Nunavut report progress on this two-page form. It records the reassessment date, number of treatments, whether function is improving, unchanged or declining, any change in diagnosis, work status, expected return-to-work dates and the job match summary. Page 2 updates the collaborative treatment plan with progress against each goal.

Who completes it

The treating physiotherapist or occupational therapist completes the whole form with the provider name, practitioner ID number and contact details. The worker and employer do not complete any part.

Submitting and payment

How to submit it
Email or fax it to WSCC's NWT or Nunavut claims department using the contacts printed on the form; WSCC's invoicing FAQ asks for medical reports within three business days of attending the worker.
Who pays
Bill treatments separately on the WSCC Medical Invoice form, sent to Financial Services within one year of the service, because the WSCC wants reports and invoices sent apart.

Good to know

  • The form asks whether a case conference is scheduled, with its date, or the reason if not.
  • Expected return to work includes modified and pre-injury duty start dates and a Disability Duration Guidelines date, alongside any flags that influence duration.
  • If Form C, the Functional Abilities Report (CS113), is not attached, explain why in the space provided.
  • Extending treatment needs prior WSCC approval on the Request for Extension (CS007), which asks for the dates of your most recent and next progress reports.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

Frequently asked questions

What changes between WSCC Form A and Form B?

Form A is the initial assessment, with family physician, employer contact and a new treatment plan. Form B reports what has happened since: number of treatments, overall functional progress, any diagnosis change, whether the worker is working, case conference status, and progress related to each goal in the collaborative treatment plan.

Do I attach Form C to the WSCC progress report?

Form B asks whether Form C, the Physiotherapy/Occupational Therapy Functional Abilities Report, is attached, and if not, why. Form C records lifting, carrying, pushing, pulling, tolerances and grip strength over up to four test periods, so it shows the functional changes that support the progress you report.

Is WSCC Form B available in French?

Yes. WSCC publishes CS112 as a fillable PDF in both English and French, and the English form notes that a French version is available. The printed revision is CS112 0824, and the same form serves workers in both the Northwest Territories and Nunavut.
