WSCC PT/OT Initial Assessment Form A (CS111)

Physiotherapists and occupational therapists in the Northwest Territories and Nunavut report their first assessment of a WSCC claimant on this two-page form. It captures the diagnosis by body part with expected recovery based on the Medical Disability Advisor (MDA), significant subjective and objective findings, employer contact, expected return-to-work dates, and a job match comparing pre-injury demands with present capability. Page 2 sets out a collaborative treatment plan.

Who completes it

The physiotherapist or occupational therapist completes the whole form, entering the provider name, practitioner name and ID number, the worker's family physician and the employer's contact details. The worker and employer do not complete any part.

Submitting and payment

How to submit it
Email or fax it to WSCC's NWT or Nunavut claims department using the contacts printed on the form; WSCC's invoicing FAQ asks for medical reports within three business days of attending the worker.
Who pays
Bill the assessment separately on the WSCC Medical Invoice form, sent to Financial Services within one year of the service, since WSCC returns invoices in other boards' formats.

Good to know

  • The job match summary rates pre-injury job requirements, present work capability and modified duties as sedentary, light, medium, heavy or very heavy.
  • The form asks whether you contacted the employer, whether job task information and modified duties are available, and whether a case conference is required.
  • If you are not attaching Form C, the Functional Abilities Report (CS113), you must say why; flags that influence recovery duration also need an explanation.
  • Follow-up uses Form B (CS112) and discharge uses the Discharge Report (CS008). The printed revision is CS111 0824, and a French version is published.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

Frequently asked questions

What is the Medical Disability Advisor on WSCC Form A?

Form A asks for a Medical Disability Advisor (MDA) diagnosis, specifying the body part, and an expected recovery date based on the MDA. It also asks whether the injury is a sprain or strain, and whether any flags or indicators influence the expected duration, with an explanation if they do.

What goes in the collaborative treatment plan on Form A?

Page 2 has rows for up to three goals, each with the methodology you will use and a recommended time frame with from and to dates. A further box covers additional requests or recommendations. Progress against these goals is then reported on Form B, the Physiotherapy/Occupational Therapy Progress Report.

Is WSCC Form A available in French?

Yes. WSCC publishes CS111 as a fillable PDF in both English and French, and the English form notes that a French version is available. The same form serves workers in the Northwest Territories and Nunavut, and the matching French Form B and Form C are published alongside it.
