WorkplaceNL Form PR: Physiotherapist's Report

Physiotherapists treating injured workers in Newfoundland and Labrador use Form PR for initial, progress and discharge reports, ticking the report type in the top right corner. It also covers recurrences, extension requests and new injuries believed to be work-related. The report records coded findings for each injured body part, functional abilities and return-to-work status, and the treatment counts WorkplaceNL uses when approving care.

Who completes it

A physiotherapist registered with the Newfoundland and Labrador College of Physiotherapists completes Sections A to F and signs the certification. The worker takes the employer's copy to the employer by the next working day.

Submitting and payment

How to submit it
WorkplaceNL's physiotherapists page says to submit Form PR through connect, its online portal, while the printed form is marked send by fax only.
Who pays
Tick 'Reporting fee requested' to have the applicable reporting fee paid under the clinic's Memorandum of Agreement; reporting fees are not paid for incomplete or illegible reports.

Good to know

  • WorkplaceNL automatically approves up to 12 treatments when it receives the initial report; requests beyond 12 must be made in writing on Form PR and need evidence of functional improvement (Policy HC-01).
  • Section B uses the code sheet effective September 2023 for body part, diagnoses, treatments, assistive devices, subjective reports and objective findings, with A to F update status codes on follow-up reports.
  • Do not stamp your name, contact details or billing number: the form is triplicate, so stamped details miss the worker and employer copies and the form is treated as illegible.
  • Mechanism of injury is needed on later reports only if it has changed, and functional abilities reflect your professional judgement, not a formal functional capacity evaluation.

Frequently asked questions

When does a physiotherapist complete WorkplaceNL Form PR?

For the initial assessment, progress reports when the worker's condition, treatment or return-to-work status changes significantly, extension requests, discharge reports, recurrences of a previous work-related injury, and new injuries the physiotherapist or worker believes are work-related. Tick the report type box in the top right corner to show which report you are sending.

Who gives the employer a copy of Form PR?

The worker. On the day of the visit you give the worker the employer's copy, and the worker must provide it to the employer by the next working day. Only sections outlined in red appear on the employer's copy. If the worker cannot deliver it in person, they must contact the employer and give the information by telephone, email or fax.

How many physiotherapy treatments does WorkplaceNL approve?

Policy HC-01 grants automatic approval for up to 12 treatments on receipt of the initial Physiotherapy Report. Continuation beyond 12 must be requested in writing on the report and is considered only where outcome measures show functional improvement with more likely, and continued treatment will keep the worker in, or return them to, the workforce.
