WorkplaceNL Chiropractor's Report (Form 8/10C)

Chiropractors treating an injured worker in Newfoundland and Labrador use this one multipurpose form for initial, progress, extension and discharge reports, ticking the report type. Findings are coded from WorkplaceNL's code sheet for body part, subjective reports, objective findings, diagnoses, treatments, investigations and assistive devices, followed by return-to-work status, a treatment summary and follow-up.

Who completes it

The chiropractor completes and signs the report, certifying it is complete and accurate with no prior payment for the visit. The worker delivers the employer's copy, which shows only the sections outlined in red, to the employer by the next working day.

Submitting and payment

How to submit it
WorkplaceNL's chiropractor page says to submit it through connect, its online service, while the PDF version is marked send by fax only. Give the worker the employer's copy on the day of the visit.
Who pays
Tick whether a reporting fee is requested; WorkplaceNL will not pay reporting fees for incomplete or illegible reports, and chiropractors view purchase orders and submit invoices through its supplier portal.

Good to know

  • Complete it for new injuries, recurrences, a significant change in condition, treatment or return-to-work status, extension requests and discharges.
  • Don't use a stamp for your name, contact details or billing number: stamps don't show on the worker and employer copies, and stamped forms are considered illegible.
  • If you are the primary health care provider, document all injured body parts, even ones you are not treating, and use Update Status codes A to F on follow-up reports.
  • An extension request records treatments to date, the number of treatments requested and their frequency, along with the stage of care and improvement since the last report.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. Frequently asked questions

When does a chiropractor complete WorkplaceNL Form 8/10C?

For new injuries the chiropractor or worker believes are work-related, possible recurrences of a previous work injury, progress when there is a significant change in condition, treatment or return-to-work status, requests to extend treatment, and discharge. The same form covers each, with the report type ticked at the top.

Why would WorkplaceNL refuse to pay a chiropractor's reporting fee?

WorkplaceNL's instructions say reporting fees will not be paid for incomplete or illegible reports, and forms completed with a chiropractor's stamp are considered illegible because stamps don't appear on the worker and employer copies. Print clearly and complete every required section.

What does the employer receive from Form 8/10C?

The employer's copy shows only the sections outlined in red, such as general information, body parts and return-to-work status, not the subjective reports, objective findings or diagnoses. The worker must deliver it by the next working day, or pass the information on by phone, email or fax if they can't do so in person.
