WorkplaceNL asks an injured worker to have their treating health care provider complete this one-page hand form and return it. It records which hand is injured and which is dominant, any previous defect, and on a diagram the site of amputations, joints with permanently restricted motion and joints permanently ankylosed, then the degrees of utmost flexion and lack of extension for each damaged joint.
The treating health care provider completes and signs the form, certifying it is complete and accurate. The worker brings it to their provider, and the worker's details, including claim number and MCP number, go at the top.

Hand Injury (Form 53)
If your copy of the form is a fillable PDF, upload it once and Vero Documents fills it from the visit note and the chart, ready for you to review and sign. Download, print, or email the finished form, with the patient’s consent.
A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

For each joint permanently damaged by the injury, record in degrees the position of utmost flexion and the lack of full extension, finger by finger for the little, ring, middle and index fingers and the thumb. Normal flexion is printed for reference: DIP 70, PIP 100 and MCP 90 degrees.
