WorkplaceNL Clinic-Based Occupational Rehab (CBOR)

Clinic-based occupational rehabilitation (CBOR) providers use this two-page report at the start, during and at the end of a WorkplaceNL worker's program. It compares initial, current or discharge and targeted tolerances, summarizes the degree of strenuousness (NOC), estimated workday tolerance and physical restrictions, tracks sessions attended and missed, and sets out the weekly plan and targeted completion date.

Who completes it

The attending licensed practitioner at the CBOR clinic completes and signs it, with the clinic name and vendor number. The worker does not complete any part.

Submitting and payment

How to submit it
Providers can complete CBOR reports online through MyWorkplaceNL (myworkplacenl.ca); the PDF version gives fax and mail as options.
Who pays
Occupational rehabilitation providers bill WorkplaceNL with the Occupational Rehabilitation Referral Invoice (OR7), which can also be submitted online through MyWorkplaceNL.

Good to know

  • Tolerances and restrictions are rated by frequency: not able, seldom (not daily), occasional (11 to 33% of a shift), frequent (34 to 66%) or constant (67 to 100%).
  • Progress reports ask whether you recommend continuing CBOR, how many more sessions, and whether the worker needs an extension to the original plan.
  • The plan section lists dates and sessions per week by program week, with original and revised targeted completion dates.
  • At discharge, record the discharge date, total sessions completed and total weeks; the current PDF is the revision of January 18, 2019.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

Frequently asked questions

Can I submit WorkplaceNL CBOR reports online?

Yes. WorkplaceNL says occupational rehabilitation providers can complete CBOR reports, submit the Occupational Rehabilitation Referral Invoice (OR7) and submit other documents online at myworkplacenl.ca, and review their submission history there. WorkplaceNL also publishes a guide and video demonstrations for completing the online CBOR report.

What goes in the CBOR summary section?

For every initial, progress and discharge report: the date of report, the degree of strenuousness using the National Occupational Classification, the estimated workday tolerance, and a list of physical restrictions with frequency ratings from not able to constant. Initial, current and targeted tolerances are compared above it.
