It covers treatment modalities and the worker's response, current DSM diagnosis, medications, substance use, suicide risk, sessions requested with a rationale, referrals to other services, and the worker's ability, confidence and desire to return to or stay at work.
The counsellor, such as a psychologist or social worker with a WorkplaceNL vendor number, completes and signs it. The worker does not complete any part.

Use the treatment details section of the progress report: give the number of sessions requested, the number provided to date and the frequency, weekly, bi-weekly or other, and a rationale for how the additional sessions will benefit the worker, with the planned modalities and interventions.
Whether the worker can return with no restrictions, with restrictions and supports, or not at all, with explanations; whether you recommend an occupational therapy assessment; barriers in presentation, functioning or affect; and the worker's confidence and desire to return to or remain at work.
