Commercial Divers Certificate of Medical Fitness (NL)

Newfoundland and Labrador's Occupational Health and Safety Regulations require a commercial diver to hold a valid certificate of medical fitness before starting any commercial diving operation. The five-page package has WorkplaceNL's instructions, the certificate, the diver's questionnaire and physical assessment, and a schedule of required tests. The certificate is renewed annually, or more often if clinically indicated.

Who completes it

A physician knowledgeable and competent in diving medicine reviews the history, examines the diver, arranges investigations and completes the declaration, classification and certificate results. The diver completes the questionnaire and the diver information section of the certificate.

Submitting and payment

How to submit it
Fax the forms to WorkplaceNL at the number printed on them; the certificate copies go to the diver (white), employer (blue), physician (pink) and WorkplaceNL (yellow).
Who pays
Under the Occupational Health and Safety Act, the diver's employer pays the medical costs unless the diver is self-employed, and the form notes such expenses are tax deductible.

Good to know

  • Classifications are fit for unrestricted diving, fit for contaminated environment, fit with restrictions, unfit temporarily or unfit permanently, with expiry one year from the examination unless otherwise specified.
  • Re-evaluate fitness whenever the diver has an event or physical condition that may affect medical status; exam reasons include after illness or accident and before returning to diving.
  • Pulmonary function testing and urinalysis are always required; chest X-ray, bloodwork, audiogram, ECG and specialist referral follow the testing schedule or clinical need.
  • The questionnaire page records that the diver's log book was reviewed and positive photo ID seen, and breast, genital, prostate, rectal and pelvic examinations are not required.
