SAAQ 7484/7484A: Information médicale complémentaire

The SAAQ sends this three-page questionnaire to the attending physician when recent medical reports say a road-accident victim cannot return to work. The answers help the SAAQ assess the overall situation, compensate the person properly and decide whether to refer them to rehabilitation. Disability is judged against a described job or, if the person had no job at the time of the accident, a presumed job based on their training and experience.

Who completes it

The attending physician answers the four questions, can use the page 3 restrictions chart, and signs both the questionnaire and the fee invoice; the SAAQ lists it among its forms for physicians and specialized nurse practitioners (IPS). The SAAQ's compensation officer completes and signs the cover letter, which carries the claim and health insurance numbers.

Submitting and payment

How to submit it
Return it through the SAAQ's online document service for external providers, by fax or by mail, and keep the original or a copy for your records.
Who pays
The SAAQ pays the professional fee entered on the attached invoice (7486 30, or 7486A 30 in English) once it receives the completed questionnaire.

Good to know

  • Question 1 asks for the limitations or functional restrictions caused by the accident injuries, each marked temporary or permanent; the chart grades lifting from under 5 kg to 20 kg or more.
  • If the patient still cannot do the job described, give your treatment plan and the expected end of the disability, or say so if you consider them fit to return.
  • Questions 3 and 4 cover obstacles to returning to work, such as a gradual return, lighter duties or bio-psychosocial factors, and any permanent inability to do the job.
  • You can ask for a phone call from an SAAQ assessing physician, and the English version (7484A 30) is reserved for health professionals without a Quebec address.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

Frequently asked questions

When does the SAAQ send the complementary medical information form?

When your recent medical reports say a road-accident victim cannot return to work and the SAAQ needs more detail on their functional limitations and ability to hold a job. The questionnaire comes with a cover letter from the compensation officer and a description of the job to consider, which may be a presumed job if the person was not employed.

Is SAAQ form 7484 available in English?

Yes, as Complementary Medical Information, Disability (7484A 30), but the SAAQ reserves it for health professionals who do not have an address in Quebec, and it notes that the original document is in French. Physicians based in Quebec use the French version, 7484 30. Both versions have the same four questions and the same functional restrictions chart.

Is the physician paid for the SAAQ complementary medical information?

Yes. The first page is an invoice for the questionnaire: enter your name, licence number and professional fees, sign it, and the SAAQ pays the fee when it receives the completed questionnaire. Send the invoice and questionnaire together through the SAAQ's online document service, by fax or by mail.
