SAAQ 5696 30: Demande de remboursement, psychologie

Private-practice psychologists and psychotherapists use this one-page SAAQ claim to be paid for a road-accident victim's psychology treatment. It records the billing period, the hours spent on the initial assessment and on treatment, and a day-by-day attendance calendar. The SAAQ asks for one each month per accident victim and first judges whether the fees relate to the accident and are relevant and necessary.

Who completes it

The psychologist or psychotherapist completes Sections 1 to 4 and signs the Section 5 declaration with their permit number. The accident victim, or their representative, signs beside it to confirm attending every listed session and to ask the SAAQ to pay the professional directly.

Submitting and payment

How to submit it
Send it every month through the SAAQ's online document service for external providers, by fax or by mail, and keep the original or a copy. The PDF can be filled on screen or printed and completed by hand, then signed.
Who pays
The SAAQ pays the psychologist or psychotherapist directly for accepted hours, entered in Section 3 as total hours times the rate, with amounts set under Quebec's Automobile Insurance Act and its regulations.

Good to know

  • Section 3 caps the initial assessment at three hours, and treatment hours need prior SAAQ authorization: the initial assessment summary (4052) unlocks up to 15 hours under balise 1.
  • Mark each day P for présence or A for absence, and tell the SAAQ about absences without delay, since only services actually given are reimbursed.
  • Before going past the authorized hours, send the progress summary (4053) at the 12th treatment hour to request balise 2, or at the 27th hour for balise 3.
  • Section 3 also asks whether treatment has ended, with a reason; paper copies of the SAAQ's psychology forms can be ordered with form 4048 30.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

Frequently asked questions

How often do you send the SAAQ psychology reimbursement request?

Every month. A note printed on the form says the SAAQ asks the psychologist or psychotherapist to send the reimbursement request monthly, and the SAAQ's forms page asks for one claim per accident victim each month. Each claim covers a billing period with from and to dates, and its attendance calendar has a row per month.

Does the SAAQ pay for missed psychology sessions?

No. The form says only services actually given are reimbursable, so mark a missed session A in the attendance calendar and inform the SAAQ of the absence without delay. For the missed appointment itself, the form tells you to apply your usual policy with the patient.

Is the SAAQ psychology reimbursement form available in English?

No. The SAAQ publishes this form, like its other forms for psychologists and psychotherapists in private practice, in French only. You can fill the PDF on screen and then print and sign it, or print it and complete it by hand. Paper copies can be ordered from the SAAQ with its order form, 4048 30.
