SAAQ 5695-30: Demande de remboursement, physiothérapie

Quebec physiotherapy clinics use this one-page French claim, backed by a page of instructions, to bill the SAAQ for treatments given to a road accident victim. It lists the treatment period, number of treatments, unit rate and total, a day-by-day attendance calendar, and declarations by the treating professional and the victim. The SAAQ decides whether the costs relate to the accident and are necessary.

Who completes it

The clinic completes the victim, supplier and billing sections, and the physiotherapist or physiotherapy technologist who gave the treatments certifies them with a licence number. The victim or their representative signs to confirm receiving the treatments and asks the SAAQ to pay the provider directly.

Submitting and payment

How to submit it
Fax the single-copy form to the SAAQ line reserved for physiotherapy reimbursement claims and progress reports. The SAAQ recommends that clinics send their claims every month, and a resubmitted claim must be marked 2e envoi ou plus.
Who pays
The SAAQ pays the clinic directly at amounts set under the Loi sur l'assurance automobile and its regulations, and only for treatments it has authorized in advance.

Good to know

  • Only the first prescription is needed to authorize reimbursement of the treatments, according to the instructions printed on the form.
  • The attendance calendar uses E for the initial assessment, P for attendance and A for absence, and the SAAQ asks to be told of absences without delay.
  • If you expect to exceed the maximum authorized number of treatments, send progress report and invoice 4056-30 at the 15th treatment or 9th week, and again at the 35th treatment.
  • Submitting the claim does not commit the SAAQ to pay; it judges the link between the costs and the accident and their relevance and necessity.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

Frequently asked questions

Does a physiotherapy clinic need a supplier number for SAAQ claims?

Yes. Section 2 asks for the supplier number, which the SAAQ assigns to each clinic. Quote it with the claim number when sending questions through the SAAQ's secure email form, choosing Indemnisation as the subject and sending one email per claim.

How often should clinics send SAAQ physiotherapy claims?

The guide printed on the form says the SAAQ recommends that the clinic send its reimbursement request every month. Each form is produced in a single copy, and if the same request is sent again, tick the 2e envoi ou plus box on the first page.

Is the SAAQ physiotherapy reimbursement form available in English?

No. The SAAQ publishes form 5695-30 in French only. It can be filled in on screen, then printed and signed, or printed and completed by hand before faxing. Clinics can order paper copies with order form 4189-35.
