Wawanesa Life Individual Life Insurance Claim Forms

Wawanesa Life does not post claim forms for its individual life insurance and critical illness policies; its individual forms page lists only policy changes such as beneficiary, address and ownership. Instead, the claimant requests a claims package from the claims team or through their insurance broker. The completed package, including any physician statement it contains, goes back to the same claims team.

Who completes it

Any attending physician statement included in the package is completed by the physician, and the claimant completes the remaining claim forms. Wawanesa Life's claims page does not list which statements the package contains.

Submitting and payment

How to submit it
The claims package is requested from Wawanesa Life's claims team by email or phone, or through an insurance broker, and completed forms are returned by email, fax or mail.

Good to know

  • The individual claims page has separate Life Insurance and Critical Illness sections, and both describe the same two steps: request a claims package, then submit the completed forms.
  • Wawanesa Life's individual forms page carries only policy-servicing forms such as change of beneficiary, change of address, change of name and transfer of ownership, not claim forms.
  • Group plan members claim differently: Wawanesa Life posts its group critical illness physician statements and group life physician's statement on its group forms and resources pages.

If your copy of the form is a fillable PDF, upload it once and Vero Documents fills it from the visit note and the chart, ready for you to review and sign. Download, print, or email the finished form, with the patient’s consent.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

Frequently asked questions

Where can I download a Wawanesa Life individual claim form?

You cannot download one. Wawanesa Life's individual life insurance claims page says claim forms are obtained directly from the claims team or through the policyholder's insurance broker. The claims package is requested by email or phone, and the individual forms page lists only policy changes, not claims.

How are completed Wawanesa Life individual claim forms submitted?

The claims page lists three channels for completed forms: email to the claims team, fax, or mail to The Wawanesa Life Insurance Company in Winnipeg. The same instructions appear in both the Life Insurance and Critical Illness sections of the page.

Is the process the same for Wawanesa Life group coverage?

No. Group plan members can download Wawanesa Life's group forms, including 33 condition-specific critical illness physician statements and a group life physician's statement, from the group forms and resources pages. The group claims page accepts those completed forms by email, fax or mail.
