RBC Insurance Disability Attending Physician Statement

RBC Insurance individual disability claims need an attending physician's statement completed by the client's physician and submitted with RBC's Disability Claim Form. RBC's claims page points physicians to the Canadian Life and Health Insurance Association's statement, and its claim form says the statement must suit the diagnosis, with the appropriate form available by calling RBC. The client opens the claim by phone first.

Who completes it

The physician completes the attending physician's statement. The client completes RBC's Individual Disability Claim Form, including a statement of disability and authorizations.

Submitting and payment

How to submit it
The statement is sent with the Disability Claim Form to RBC Life Insurance Company's Life and Health Claims Department by email or fax.

Good to know

  • RBC's claim form says the completed forms must reach RBC Life Insurance Company within 90 days of the claimed disability date.
  • RBC's document checklist accepts either the attending physician's statement or the available medical records.
  • The client should call RBC before sending anything; RBC asks clients to make a claim request by phone, with the policy number if they have it.
  • The claim form also asks for every physician and health care provider treating the condition, including physiotherapists, chiropractors, psychologists and counsellors.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.
