Desjardins Brand-Name Drug Request (12126E)

Desjardins group plans cover a brand-name drug only up to the price of the lowest-cost generic equivalent, and this two-page request asks for an exception. It is approved only if the attending physician gives an acceptable medical reason the patient cannot take that generic: a documented allergy or severe intolerance, including the non-medicinal ingredient that caused it. Desjardins' physician or pharmacist may review it.

Who completes it

The plan member completes sections A and B, with a separate signature from any insured dependent aged 16 or over; the attending physician completes section C, the physician's statement, and section D, identification and signature.

Submitting and payment

Good to know

  • Only an allergy or intolerance is accepted as the observed reaction; the form states any other justification will be denied.
  • If a generic without the offending non-medicinal ingredient is available in Canada, the request is denied, since the brand must be the only option without it.
  • Describe the reaction's outcome, such as hospitalization, prolonged hospitalization, emergency intervention or persistent disability, with its onset and the treatment given.
  • The French version of the form is numbered 12126F, and the English 12126E is dated 2026-04.

The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.
