Desjardins 02026A: Additional Physician Report

Desjardins Insurance uses this two-sided follow-up report for ongoing group disability claims: one side for physical illnesses, the reverse for psychological illnesses. It updates the insurer on diagnosis, treatment, functional limitations, prognosis and the expected return to work. The form, dated 2018-09, carries the logos of the Fédération des médecins omnipraticiens du Québec and the Regroupement des assureurs de personnes à charte du Québec.

Who completes it

The employee completes Section 1 with their policy, group or contract and certificate numbers. The attending physician completes the side that matches the condition, in block letters, and gives the form to the employee.

Submitting and payment

How to submit it
The employee sends the completed report to Desjardins Insurance online, by mail or by fax, using the options printed at the top of the form.
Who pays
The form states in capitals that the employee must pay the fees requested to complete it.

Good to know

  • If you expect the absence to exceed the usual period for the diagnosis, the form asks you to specify the factors that justify your prognosis.
  • The psychological side follows DSM-IV axes: personality disorders and addictions, associated illness, psychosocial stressors in the last 12 months, and GAF scores at the start of treatment and now.
  • Both sides ask about the patient's cooperation with treatment (excellent, average or poor) and whether they would benefit from help returning to work.
  • Physical findings include weight, height, latest blood pressure and symptom severity graded mild, moderate or severe, with recent results, X-rays or ECGs attached.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

Frequently asked questions

When is the Desjardins additional report used?

During an ongoing Desjardins group disability claim, after the initial attending physician's statement (02025A). It records the last consultation, tests to come, follow-up frequency, specialist referrals, how functional limitations have changed since the start of the disability, and how long before the patient can return to work.

Can the Desjardins additional report be filled in online?

Partly. The PDF is fillable, but the diagnosis section is not fillable online because Desjardins says diagnosis information should come from the attending physician. Complete that part in block letters and give the form to the employee, who sends it online, by mail or by fax.

Who pays for the Desjardins additional report?

The employee. Both the physical and psychological sides state in capitals that the employee must pay the fees requested to complete the form. The physician completes the diagnosis and follow-up sections in block letters and gives the form to the employee, who sends it to Desjardins.
