Desjardins 02025A: Initial Physician Statement

Desjardins Insurance's initial attending physician's statement for group disability claims has a physical-illness version on one side and a psychological-illness version on the reverse. It records the diagnosis, treatment, follow-up and expected length of the disability at the start of a claim. The French version, Déclaration initiale du médecin traitant, is numbered 02025; the English 02025A is dated 2018-09.

Who completes it

The employee completes Section 1 with their policy and certificate numbers. The attending physician completes the version that matches the condition, in block letters, and gives the form to the employee.

Submitting and payment

How to submit it
The employee sends it to Desjardins Insurance online, by mail or by fax, using the options printed at the top of the form, and saves the PDF on a computer before submitting online.
Who pays
The form states that the employee must pay the fees requested to complete it.

Good to know

  • The physical side asks whether the disability relates to an accident, illness, occupational or automobile accident, pregnancy or preventive withdrawal from work, with the event or delivery date.
  • The psychological side grades symptom severity, including with psychotic elements, and asks whether work stopped because of family, job loss, professional, personal or substance-related problems.
  • Desjardins' group guide recommends sending short-term disability claims within 10 days of the last day worked, and long-term claims at least four weeks before eligibility.
  • Desjardins' current group claims page and guide ask for three documents: the Employee Statement (06329E), the Employer Statement (08317E) and a doctor's note.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

Frequently asked questions

Which side of the Desjardins statement should I complete?

The side that matches the condition: the physical illness version, or the psychological illness version on the reverse. Both end with the approximate duration of the disability in days or weeks and how long before the patient can return to work, part-time, full-time or gradually.

How long does Desjardins take to process a group disability claim?

Once all required documents arrive, Desjardins' plan member guide gives usual processing times of 5 business days for short-term disability and 10 business days for long-term disability. A claims specialist may call the employee, and the decision is confirmed in writing.

Does Desjardins share medical details with the employer?

No. Desjardins' group disability guide says it shares only whether the employee is eligible for benefits, the return-to-work plan, and what the employer needs to know to make the return easier, such as functional limitations. The diagnosis on this statement stays with the insurer.
