Desjardins 020265A: Physician's Additional Report

Desjardins Insurance asks the attending physician for this additional report during an ongoing individual disability insurance claim. Like the group version, it has a physical-illness side and a psychological-illness side, but it adds a section for questions specific to the insured's contract. The English version is 020265A (2020-11); the French Déclaration du médecin traitant, Rapport supplémentaire, is numbered 020265.

Who completes it

The insured completes the identification section with contract, group or policy, and certificate numbers. The attending physician completes the declaration on the matching side, in block letters, and gives it to the patient.

Submitting and payment

How to submit it
The physician gives the completed report to the patient, who submits it to Desjardins Insurance online or by mail.
Who pays
The form states that the insured must pay the fees requested to complete it.

Good to know

  • Section 4 is titled Questions specific to the contract; answer anything listed there in addition to the standard diagnosis, treatment and follow-up questions.
  • The psychological side asks about care by a psychiatrist, psychologist or social worker, in a CLSC, day hospital or therapy group, and uses DSM-IV axes.
  • Only a few fields in the PDF are fillable, so most of the declaration is written by hand in block letters.
  • Both sides ask whether the absence will exceed the usual period for the diagnosis and, if so, which factors justify that prognosis.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

Frequently asked questions

What is the difference between Desjardins' 020265A and 02026A?

Both are follow-up physician reports with physical and psychological sides. The 020265A is for individual disability insurance and includes a section of contract-specific questions, while the 02026A is for group disability claims, where the employee receives the completed form.

Who pays for the Desjardins additional report?

The insured. The form states that the insured must pay the fees requested to complete it, and Desjardins' individual claims page also says the insured pays any fees the doctor charges. The physician gives the completed form to the patient to submit.

Does the psychological side ask for a functioning score?

Yes. Axis V asks for a functioning score on the DSM-IV scale from 0 to 100, where 100 is perfect condition, at the beginning of treatment and currently. The same side covers personality disorders, addictions, associated illness and psychosocial stressors in the last 12 months.
