Desjardins 14153E: Death Claim Physician's Statement

Desjardins Insurance uses this one-page statement when a group life insurance death claim needs the physician's account of the death. It records the place and date of death, the condition directly leading to death and its antecedent causes, attendance and diagnosis dates, whether an inquest or autopsy took place, five years of treatment history and tobacco use.

Who completes it

The attending physician completes Section B (physician's statement) and Section C (identification, licence number, general practitioner or specialist, and signature). The claimant fills in the employee and contract details and Section A on the deceased.

Submitting and payment

How to submit it
Desjardins lists the form with its death claim forms and asks claimants to send completed forms and required documents by secure online upload or by mail.
Who pays
The form states that fees charged for this statement are to be paid by the claimant.

Good to know

  • Question 1 asks for the disease, injury or complication that caused death, not the mode of dying such as heart failure or asthenia, with the interval between onset and death.
  • The physician's statement section cannot be filled in online, because Desjardins wants the diagnosis information to come from the attending physician.
  • Questions 11 and 12 ask whether you, or any other physician or hospital, treated the deceased in the 5 years before the last illness, with dates and details.
  • The French version is 14153F; the claim also uses the beneficiary's statement (02227A) and the employer's statement (12123E).

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

Frequently asked questions

Can the Desjardins death claim physician's statement be filled in online?

Not the physician's part. The form notes that information about the diagnosis should be provided by the attending physician, so that section cannot be filled in online. The physician completes the statement and identification sections and signs, indicating whether they are a general practitioner or a specialist.

What causes of death does Desjardins form 14153E ask for?

Question 1 asks for the disease or condition directly leading to death, meaning the disease, injury or complication rather than the mode of dying. Question 2 lists antecedent causes, and question 3 other significant conditions that contributed to the death without being related to the condition causing it.

Who pays for the Desjardins death claim physician's statement?

The claimant. A notice on the form states that fees charged for this statement are to be paid by the claimant. The completed statement accompanies the beneficiary's and employer's statements, and Desjardins accepts claim documents by secure online upload or by mail.
