Desjardins 17026A: Critical Illness Physician Statement

Desjardins Insurance uses this three-page statement for the medical part of a critical illness claim under individual, GetWell or group insurance. Because benefits depend on the illness meeting the contract's specific criteria, it asks for detailed diagnosis dates, five years of treatment history, tobacco use and hereditary family history, plus condition-specific details for cancer, heart attack, stroke or another illness.

Who completes it

The insured's attending physician, or the specialist who diagnosed the critical illness, completes Sections B to E, including identification with licence number and signature. The insured completes Section A, indicating whether the coverage is individual, GetWell or group insurance.

Submitting and payment

How to submit it
Desjardins asks claimants to fill out the forms, gather the required documents and send them by secure online upload or by mail, with the requested records enclosed with this statement.

Good to know

  • For cancer, enclose the complete medical file with the pathology report for the diagnostic biopsy, and give the cancer site, TNM or numeric stage and whether it is a recurrence.
  • For heart attack, Section C asks about diagnostic rises and falls in cardiac markers, new ECG changes, new Q waves after a cardiac procedure, and whether it is a first infarction.
  • For stroke, state whether neurological deficits persisted more than 30 days after the diagnosis, describe them, and say whether the stroke was caused by trauma.
  • The French version is numbered 17026; companion forms include the insured's critical illness claim and, for group claims, the employer's statement (12123E).
