BMO Insurance Life and Critical Illness Claim Forms

BMO Insurance does not publish blank physician statements for life or critical illness claims. The insured, a beneficiary or the estate's executor starts the claim online or by phone, BMO sends the relevant claims package, including any physician statement, and the claim is evaluated for a final decision once all required documentation has been returned.

Who completes it

The attending physician completes whatever physician statement BMO includes in the claims package. Only the insured, a beneficiary or the executor of the estate can start the claim, giving the insured's name, the policy number and the type of claim.

Submitting and payment

How to submit it
The claim starts with BMO's online claim form or a call to BMO during weekday business hours, after which BMO sends the claims package to be completed and returned.

Good to know

  • BMO's online claim form offers two claim types, death or other (to be specified), and records the requester's relationship to the insured: the insured, a beneficiary, a spouse or other.
  • Claims on creditor insurance for a BMO loan, mortgage or line of credit go through BMO's separate Creditor Insurance site, not this life and critical illness process.
  • BMO says there is no mandated time frame for investigating a claim in Canada, and that its claim teams strive to respond promptly.
  • BMO encourages people with questions about the claims process to contact their advisor or BMO's claims line, which is staffed Monday to Friday.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

Frequently asked questions

Where can I download a BMO Insurance critical illness physician statement?

BMO does not post one on its claims page. The insured, beneficiary or executor starts the claim through BMO's online form or by phone, and BMO then sends the relevant claims package to be completed. Any physician statement your patient brings will come from that package.

Who can start a BMO life or critical illness claim?

BMO says you must be the insured, the beneficiary or the executor of the estate to make an insurance claim. The online form asks for the requester's name, relationship to the insured and contact details, plus the insured's name, the policy number and whether the claim is for a death or another type.

How long does BMO take to decide a claim?

BMO states there is no mandated time frame for investigating a claim in Canada, and that its claim teams strive to respond promptly. Once all required documentation from the claims package is returned, the claim is evaluated for a final decision, and settlement is delivered in the claimant's preferred method.
