RBC LoanProtector/HomeProtector Disability Claim (3503)

RBC Royal Bank clients with LoanProtector or HomeProtector disability coverage on a Royal Credit Line, personal loan or mortgage claim with this 10-page package. Canada Life is the insurer, and RBC's Insurance Service Centre forwards the claim once all documents are in. The clinician completes the embedded Initial Disability Insurance Medical Statement, the industry-standard form, June 2025 version.

Who completes it

The claimant completes the Disability Benefit Claim Form, the authorizations and Section 1 of the medical statement, and the current employer completes the Employer Statement. The licensed physician treating the claimant completes Section 2 of the medical statement.

Submitting and payment

How to submit it
The completed forms go by mail to RBC Insurance Services' Insurance Service Centre, which cannot forward the claim to Canada Life until every required document has arrived.
Who pays
The claimant is responsible for securing the medical statement and for any charge for its completion.

Good to know

  • No benefits are paid for the first 60 consecutive days, so RBC tells claimants not to submit unless the disability will last longer than 60 days.
  • The claim must reach the insurer within 150 days of the start of disability or it will be denied; a late-claim exception appeal needs all medical information since the disability began.
  • Canada Life may need additional medical history from the physician, and will advise in writing if more medical proof is required.
  • While the claim is processed, the borrower keeps payments up to date and should not close or refinance the loan or mortgage.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

Frequently asked questions

When should a patient submit an RBC LoanProtector disability claim?

Only once it is clear the disability will last longer than 60 consecutive days, because no benefits are paid during that waiting period; then as soon as possible. The claim must reach Canada Life within 150 days from the day the disability started, otherwise it will be denied.

Which insurance does Form 3503 cover?

LoanProtector (group policy H28445) and HomeProtector (group policy H60101) disability coverage on an insured Royal Credit Line, personal loan or mortgage. Canada Life issued these group creditor policies to Royal Bank of Canada and its associated mortgage and trust companies; benefits are paid to RBC on the borrower's behalf.
