Frequently asked questions

Can the physician send Part B directly to TD Life?

Yes. The instructions say Part B can be detached and given to the attending physician to complete and send separately to TD Life Insurance. Otherwise the claimant returns the original forms, Part A and Part B, to TD Insurance's Claims Department and keeps a photocopy.

Should genetic test results be included with the TD medical statement?

No. TD's instructions say the request for medical records excludes genetic test results and asks that none be provided. The medical statement itself also notes that the medical and health information released excludes genetic test results.
