RAMQ 8500: Nexium / Prevacid FasTab PMP Exemption

This 2-page request asks RAMQ to exempt Nexium (esomeprazole) or Prevacid FasTab (lansoprazole) from the maximum payable price for a patient who can only take a proton pump inhibitor in dissolved form. It applies to severe dysphagia or to a nasogastric or gastrojejunal tube. RAMQ no longer updates the form and points prescribers to its online service instead.

Who completes it

The authorized prescriber completes every section: patient and prescriber details, the drug with strength, dosage and planned duration, the clinical reason in section 4, then signs section 6 certifying the information is accurate.

Submitting and payment

How to submit it
RAMQ suggests making the request through its online service Patient et médicaments d'exception. The PDF can still be returned by fax or mail, but RAMQ warns that this version adds processing time.

Good to know

  • Eligibility requires that the patient can take a proton pump inhibitor only in dissolved form, plus severe dysphagia, a nasogastric tube, or a gastrojejunal tube.
  • For severe dysphagia, section 4 asks you to specify the cause; for a gastrojejunal tube, indicate whether it is temporary or permanent.
  • Treatment duration can be marked indeterminate or given as start and end dates. If the patient is in hospital, note the planned discharge date.
  • Separately, RAMQ reimburses Prevacid FasTab 15 mg and 30 mg without a form when Ne pas substituer is written for a patient under 18 who needs an orally disintegrating tablet.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

Frequently asked questions

Is RAMQ form 8500 still in use?

RAMQ still lists form 8500 on its professional forms index, but the PDF states that RAMQ no longer updates it. Prescribers can make the same request through the online service Patient et médicaments d'exception. If you send the PDF version instead, expect additional processing time.

Which patients qualify for a PMP exemption for Nexium or Prevacid FasTab?

Patients who can take a proton pump inhibitor only in dissolved form and who have severe dysphagia, a nasogastric tube, or a gastrojejunal tube. Section 4 of the form records which situation applies, with the cause of the dysphagia or the type of tube, and section 5 leaves room for optional extra details.

Is form 8500 available in English?

RAMQ publishes form 8500 in French only, under the title Demande d'exemption du prix maximum payable (PMP). Before returning it, RAMQ asks prescribers to confirm that all required sections are completed and that the form is signed, since unsigned or partial forms slow the request.
