RAMQ 4596: Innovator (Brand-Name) Drug Reimbursement

When a Québec patient reacts badly to a generic and needs the brand-name (innovator) version, this 2-page request asks RAMQ to reimburse the innovator rather than only the lowest-priced equivalent. Approval requires a serious allergy or intolerance to a non-medicinal ingredient the innovator doesn't contain. Without approval, the public plan pays the lowest price and the patient pays the difference.

Who completes it

The prescriber completes the whole form: insured person, prescriber, the innovator drug requested, the questions about the reaction in section 4, and the signed certification in section 5 that the information is accurate.

Submitting and payment

How to submit it
Save the completed form and send it, with any documents supporting the request, through RAMQ's secure upload page.

Good to know

  • Lack of efficacy of generic versions is not an accepted justification. The form accepts only allergy or intolerance, and RAMQ refuses any other reason.
  • The intolerance must have led to hospitalization, a longer hospital stay, emergency medical intervention, or a persistent or significant disability or incapacity.
  • You must check product compositions yourself. If another version sold in Canada lacks the ingredient that caused the reaction, RAMQ refuses the request.
  • No form is needed for some drugs prescribed with Ne pas substituer, such as Prevacid FasTab for patients under 18 or DDAVP Melt, when RAMQ's listed conditions are met.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

Frequently asked questions

Does writing Ne pas substituer get the brand-name drug covered in Québec?

Not on its own. The public plan reimburses the lowest-priced equivalent, and the patient pays the difference for the innovator. Only a short list of drugs is reimbursed with the Ne pas substituer mention, under specific conditions, without a form. In other cases the prescriber sends form 4596 showing a serious reaction to a generic's non-medicinal ingredient.

What reaction details does form 4596 ask for?

Section 4 asks for the generic brand responsible, whether the reaction was an allergy or an intolerance, its consequence, a detailed description and treatment, the date, the non-medicinal ingredient involved, whether that ingredient is in the innovator, whether a suitable generic exists in Canada, and any other molecule tried.

Will RAMQ process an incomplete form 4596?

No. The form states that every requested item must be provided and that RAMQ will refuse an incomplete request. Answer all of section 4, including the questions about generics available in Canada, and send any supporting documents with the form through the secure upload page. Section 4 question 10 is the only optional item.
