RAMQ 3633: Exception Drug (Médicament d'exception)

Québec prescribers use this request when a drug listed in Annex IV of the Liste des médicaments needs RAMQ payment authorization and no exception-drug code fits the patient. More than 40% of exception drugs are codified, so a code on the prescription often replaces the request. When it doesn't, RAMQ checks the justification against the drug's recognized indications for payment.

Who completes it

An authorized prescriber completes the whole form: insured person, prescriber with RAMQ registration number, the drug requested, the justification and diagnosis, then signs it. In the online service, the prescriber can delegate entry to administrative or nursing staff once RAMQ grants them access.

Submitting and payment

How to submit it
Submit through RAMQ's online service Patient et médicaments d'exception, and fax the PDF form only if you have no access to the service or run into technical problems. RAMQ sends its decision to the prescriber's secure messaging.

Good to know

  • Use one form per drug. Section 3 asks for the drug name, form, strength, dosage, planned treatment dates, and whether it will be given at home, in a CLSC, an office or a facility.
  • RAMQ's service statement sets 1 business day for a complete priority exception-drug request and 25 business days for any other complete request; some files need longer, complex review.
  • For codified drugs, write the code again on every renewal. The code stays valid while you remain the prescriber; write XX on the prescription to tell the pharmacist it no longer applies.
  • After a refusal, the patient can request a review within 6 months, and you can file it for them as a new request with new information. The Tribunal administratif du Québec comes next.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

Frequently asked questions

Do I need form 3633 if the exception drug has a code?

No. For many exception drugs, codification replaces the payment authorization request: you write the code matching the patient's recognized indication on the prescription, and coverage is confirmed without delay. Use a code only when it matches the patient's situation. The online service shows each drug's code and indications when you start a new request.

Is RAMQ form 3633 available in English?

RAMQ publishes form 3633 in French only. The online service Patient et médicaments d'exception is RAMQ's main channel for these requests, and the PDF is meant for prescribers without access or with technical problems. Prescribers outside Québec who haven't joined Québec's health insurance plan use form 4515 instead.

How long does RAMQ take to decide an exception drug request?

RAMQ commits to answering a complete priority exception-drug request within 1 business day and any other complete request within 25 business days, although some files need complex processing. Incomplete requests take longer, so include every required supporting document. The decision arrives in the prescriber's secure messaging.
