IVAC 280: Attestation d'un intervenant ou professionnel

A victim's request to IVAC must come with a supporting document, and this attestation is one accepted way to provide it. The support worker or professional confirms that the person disclosed a criminal offence to them and records its description, date and place. IVAC also accepts other documents instead, such as a health evaluation or medical record from a member of a professional order, or a police report.

Who completes it

The person applying fills Section 1, and the support worker or professional completes and signs Section 2: a victim-support worker, police officer, legal worker or health professional who is a member of a professional order.

Submitting and payment

How to submit it
It is joined to the victim's IVAC request, the Demande de qualification (F_255 or F_254); the form shows the fax details for sending it.

Good to know

  • Section 2 records what the victim disclosed to you: a description of the criminal offence, its date and its place.
  • It is one option: a health evaluation or medical record from a member of a professional order, or a police report, can serve as the mandatory supporting document instead.
  • The victim's request is filed on Demande de qualification F_255 (age 14 and over, or adults) or F_254 (minors or incapable adults).
  • The current version is dated 2026-06: a fillable PDF in French only, completed on screen and then signed.

Frequently asked questions

Who can sign an IVAC attestation for a crime victim?

A victim-support worker, a police officer, a legal worker or a health professional who is a member of a professional order. They complete Section 2, attesting that the victim disclosed the criminal offence and giving its description, date and place, after the person applying has filled in Section 1.

Is the IVAC attestation mandatory?

A supporting document is mandatory with an IVAC request, but this attestation is only one way to provide it. IVAC also accepts a health evaluation or medical record from a member of a professional order, or a police report. The chosen document goes with the victim's Demande de qualification.

Is IVAC form 280 available in English?

No. Like IVAC's other forms, the Attestation d'un intervenant ou d'un professionnel is published in French only. The current fillable PDF is dated 2026-06; it can be completed on screen, then signed and sent with the victim's request using the fax details printed on the form.
