IVAC 257: Rapport d'évaluation initiale, psychothérapie

After the first sessions with a crime victim or rescuer, the treating psychologist, psychotherapist or psychosocial professional uses this report to give IVAC an overall picture of the person's situation in relation to the recognized offence or rescue. IVAC publishes it as a Word template with fill-in fields, and pays for it only when it asked for the report and the template was used in French.

Who completes it

An IVAC-registered psychologist, psychotherapist holding a permit, or professional authorized for psychosocial follow-up completes the whole report. The sessions themselves are billed separately on the Compte de services (258).

Submitting and payment

How to submit it
Send it to the Direction générale de l'IVAC online, by mail or by fax, within 15 days of IVAC's request.
Who pays
IVAC reimburses the time spent writing the report at up to the same hourly cap as sessions, but only when it requested the report, the IVAC template was used and the report is in French.

Good to know

  • IVAC's forms page lists the template as Rapport d'évaluation psychologique (F_257); it is a Word .docx file with fill-in fields, not a PDF.
  • A report written on another template, or not written in French, is not reimbursed.
  • Follow-up reporting uses the Rapport d'évolution (256) and, when follow-up ends, the Rapport final (259), on the same terms.
  • The current template is dated 2022-10 and covers both victims of a criminal offence and rescuers.

Frequently asked questions

When does IVAC need an initial evaluation report?

When IVAC requests it, after the first sessions with a victim or rescuer. The report gives IVAC an overall picture of the person's situation in relation to the recognized offence or rescue, and it is due within 15 days of the request. IVAC reimburses it only when it asked for the report.

Can I use my own report format for IVAC?

Not if you want to be paid. IVAC reimburses the initial evaluation report only when it requested the report, the IVAC template was used and the report is written in French. The template is a Word document with fill-in fields, listed on IVAC's forms page as Rapport d'évaluation psychologique (F_257).

How is report writing paid by IVAC?

At up to the same maximum hourly rate IVAC pays for sessions, when IVAC requested the report. Bill the time on the Compte de services d'intervention psychothérapique ou psychosociale (258), along with the session dates and hours, and send it to IVAC online, by fax or by mail.
