IVAC 229: Rapport final (crime victims)

When a crime victim's or rescuer's injury is consolidated, the physician providing medical follow-up completes this final report for IVAC. It gives the consolidation date and states whether permanent sequelae or functional limitations remain; if they do, a Rapport d'évaluation médicale (REM) on CNESST form 2064 follows. IVAC expects one final report per victim or rescuer, at the end of the consolidation period.

Who completes it

The physician providing medical follow-up, or a specialized nurse practitioner (IPS), completes the whole form.

Good to know

  • Only one final report is completed per victim or rescuer, at the end of the consolidation period.
  • A yes on permanent sequelae or functional limitations calls for a REM, completed on CNESST's Rapport d'évaluation médicale (2064), which IVAC uses for victims too.
  • The Rapport final follows the Attestation médicale (227) and any Rapport médical (228) in IVAC's medical file.
  • The fillable PDF is dated 2019-09 and published in French only; IVAC's medical forms guide (DC200-1007-3) explains how to complete it.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

Frequently asked questions

When do I complete the IVAC Rapport final?

At consolidation, when the injury of the victim of a criminal offence or the rescuer has stabilized. The report gives the consolidation date and says whether permanent sequelae or functional limitations remain. IVAC expects one final report per victim or rescuer, at the end of the consolidation period.

What happens if sequelae remain after the IVAC final report?

A Rapport d'évaluation médicale (REM) is then needed, completed on CNESST's form 2064, which IVAC uses for victims too. It is billed under RAMQ code 09944, separate from the final report's code, 15797. The REM covers the diagnosis, examination, permanent functional limitations and sequelae in detail.

How is the IVAC final report billed?

Through RAMQ with code 15797, which is printed on the form. Physicians who want IVAC to pay them directly register as IVAC suppliers, as for the Attestation médicale (227) and Rapport médical (228). Keep a copy for your file and send IVAC's copy online, by fax or by mail.
