IVAC 259: Rapport final, psychothérapie (crime victims)

When psychotherapeutic or psychosocial follow-up with a crime victim or rescuer ends, the treating professional sends IVAC this final report. It sets out the conclusions, the person's own perception, the results against the treatment objectives and the reasons for ending. When no significant improvement remains possible, it can support a lump-sum claim for permanent sequelae or temporary impairment.

Who completes it

An IVAC-registered psychologist, psychotherapist or professional authorized for psychosocial follow-up completes the whole report on IVAC's Word template.

Submitting and payment

How to submit it
Send it to the Direction générale de l'IVAC online, by mail or by fax, within 15 days of IVAC's request.
Who pays
IVAC reimburses report writing at up to the same hourly cap as sessions when it requested the report, billed with the sessions on the Compte de services (258).

Good to know

  • The report compares results with the treatment objectives, gives the person's own perception of their situation, and explains why follow-up is ending.
  • When no significant improvement is still possible, the report can support a lump-sum indemnity for permanent sequelae or temporary impairment.
  • IVAC's forms page lists it as Intervention psychologique, Rapport final (F_259), a Word .docx template with fill-in fields dated 2022-10.
  • Reimbursement requires an IVAC request, IVAC's template and a report written in French, as for the initial (257) and progress (256) reports.

Frequently asked questions

What goes in the IVAC final psychotherapy report?

The conclusions of the follow-up, the person's own perception of their situation, the results achieved against the treatment objectives and the reasons for ending. IVAC publishes it as a Word template with fill-in fields (F_259), and it must be written in French on that template to be reimbursed.

Can the IVAC final report support a lump-sum indemnity?

Yes. When no significant improvement remains possible, the final report can support the victim's or rescuer's claim for a lump sum for permanent sequelae or temporary impairment. Send it to the Direction générale de l'IVAC within 15 days of IVAC's request, online, by mail or by fax.

Is the IVAC final psychotherapy report paid?

When IVAC requested it, report writing is reimbursed at up to the same maximum hourly rate as sessions; bill the time with your sessions on the Compte de services (258). A report on another template, or not written in French, is not reimbursed even after a request.
