Wyoming Health Care Provider Initial Medical Report

Wyoming's first medical report on a work injury comes from the provider who first treats the worker, covering history, exam findings, ICD-10 diagnosis, work-relatedness and work status. It satisfies the initial provider report required by W.S. 27-14-501, and an injury report must be on file before the Division pays benefits to the worker or the provider. The claims analyst contacts the provider while deciding whether to open the claim.

Who completes it

The initial treating health care provider (MD, DC, OD or other) completes the whole form, including the employee's first-person statement of cause and complaints, and signs it with an original signature. The employee and employer file their own Report of Injury separately.

Submitting and payment

How to submit it
Return the report to the Workers' Compensation Division in Cheyenne, with the claim number if known. Providers can follow claim and billing status online in the Division's PIERS portal.
Who pays
No fee is printed on the form; the visit is billed directly to Wyoming Workers' Compensation with the claim number and medical documentation for each date of service, and in-state providers cannot bill the worker above the Division's fee schedule.

Good to know

  • The form notes that Wyoming workers' compensation is exempt from HIPAA regulations, and the Division also posts a HIPAA letter for providers on its forms page.
  • Item 18 requires ICD-10 codes, and item 20 asks whether the condition is the result of the injury described in item 15: yes, no or undetermined.
  • Disposition items cover medical stationary status, further treatment, permanent impairment, time loss and modified-duty restrictions; a worker who cannot work applies for TTD separately on Form WCD-9.
  • Chiropractors may substitute computerized notes titled 'Initial Exam' when they include all the required information, under the Division's chiropractic utilization guidelines.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Vero is not affiliated with, or endorsed by, the agencies and insurers listed here. Read our editorial policy.

Frequently asked questions

Can I treat a Wyoming injured worker before there is a claim number?

Yes. DWS tells injured workers they can get treatment even without a claim number. Once the worker's Report of Injury is received, the Division assigns a nine-digit claim number, which the worker gives to every provider so bills go directly to Workers' Compensation. Leave the claim number blank on the report if unknown.

Who can complete the Wyoming initial medical report?

The initial treating health care provider, whose name and type (such as MD, DC or OD) go in item 32. Wyoming workers may choose a primary provider who is a doctor of medicine, chiropractic or osteopathy, an optometrist, podiatrist, psychologist or advanced practitioner of nursing.

Does the initial report count as an application for disability benefits?

No. It reports the injury and your findings, including whether the injury prevents regular work and an estimate of time loss. The Division treats claims for benefits separately, so a worker seeking temporary total disability files Form WCD-9, whose provider certification you complete after examining the worker.
