Wyoming DME Preauthorization Request (Workers' Comp)

Wyoming's Workers' Compensation Division uses this one-page request to review higher-cost durable medical equipment before it is supplied to an injured worker. It applies only above the Division's dollar threshold; lower-cost DME needs no preauthorization and is billed straight to CorVel. A DME coordinator nurse completes the bottom block with the decision, a preauthorization number and the dates the approval is valid.

Who completes it

The requesting provider or DME supplier fills in the claimant, claim number, requestor's federal tax ID and contacts, and lists each item with HCPCS code, description, quantity, quote and time frame. The treating provider supplies the order or letter of medical necessity, and only the Division's DME coordinator nurse completes the authorization block.

Submitting and payment

How to submit it
Fax the form to the Division's RN DME coordinators with the treating provider's order or letter of medical necessity, the diagnosis codes and the clinical records supporting the request.
Who pays
The request carries no fee; approved equipment is billed afterward, and every DME bill goes through bill review under Chapter 10, Section 8 of the Wyoming Workers' Compensation Rules.

Good to know

  • Preauthorization in Wyoming is voluntary; the Division currently reviews only surgical procedures and DME above its threshold, under its Rules Chapter 10, Section 23.
  • Since September 1, 2021, the Division no longer preauthorizes lower-cost DME; those items are billed directly to CorVel and still go through bill review.
  • Home and vehicle modifications use a separate Home and Vehicle Modification Preauthorization Request form, and the preauthorization page also posts DME policy bulletins such as TENS and CPAP/BiPAP.
  • The approval block records the authorization date, a preauthorization number, the dates the approval is valid from and through, and the coordinator nurse's signature and comments.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Does all DME need preauthorization in Wyoming workers' comp?

No. The Division reviews DME only above its preauthorization price threshold, and only on a voluntary basis. Since September 1, 2021, lower-cost DME needs no preauthorization and is billed directly to CorVel. All DME bills are still reviewed under Chapter 10, Section 8 of the Division's rules.

What must come with a Wyoming DME preauthorization request?

The Division requires the treating health care provider's order or letter of medical necessity, the diagnosis codes, and clinical medical documents supporting the request. On the form itself, list each item with its HCPCS code and description, the quantity, the quote and, where relevant, the time frame of use.

Who approves a DME request in Wyoming?

The Division's RN DME coordinators review the request. The bottom section is reserved for the coordinator nurse, who marks the authorization, signs, adds comments and assigns a preauthorization number along with the dates the approval is valid.
